dried PBW + cloudy StarSan

I washed a big batch of bottles this morning.
Emptied the PBW out of all of them at the same time; by the time I got to the last ones for rinsing, the PBW had dried - white residue visible outside + presumably inside.

On the interwebs, I can't find any agreement on how to sort this:
normal StarSan or vinegar solution as an acid wash, undiluted StarSan, or (from 5 Star themselves when I mailed them) complete rewash with a bottle brush.

Anybody have a good way of removing dried PBW from inside bottles?

As a side quest from this thread, I also got to wondering when StarSan needs to be dumped.
Some say that if it goes cloudy, it's reacted with minerals and the surfactants aren't working anymore.
Others (again, including the manufacturer) say that as long as the ph is under 3.4, you're good.

Is cloudy StarStan still good if the ph stays low?

My StarSan goes cloudy as soon as I mix it if I use tap water so something in my water is reacting. I just checked the pH of a batch I made at the start of December and it's showing 2.8

That being said, I mix up a fresh batch every couple of brews as I found it leaves a film on things the longer it is kept around.

I had this problem with a glass carboy which I soaked way to long in Oxy Clean, resulting in a thick white residue at the inside of the carboy.

I swirled around a cup of phosphoric acid (75%) which worked a treat.

Be careful using this stuff. Glasses and gloves.

StarSan at regular (diluted) strength did not shift the residue on the outsides of the bottles. At all.  :(
